Thank you for your interest in participating in the Faces of Breast Cancer portrait and storytelling campaign, hosted by Aurora BayCare in collaboration with STUDIO NÜDE BOUDOIR.

What’s included:
💄 1-hour hair and makeup session
📝 Interview during makeover
📸 30-minute boudoir-style photo session
🖼️ 3–5 edited digital photos + 1 featured portrait
🌐 Story and portrait published on BayCare Clinic’s website and social media

Session Dates: August 11–13, 2025
Time Slots:
10:00–11:30 AM
12:00–1:30 PM
1:45–3:15 PM
3:30–5:00 PM

Please select your first and second choice time slots. Final participants will be selected randomly. You must be available during your selected times.

By submitting this form, you agree to:
- Participate in a boudoir-style photo session (nudity not required)
- Share your breast cancer journey in an interview
- Allow BayCare Clinic and Studio Nude to publish your story and portrait
- Sign a media release and participation agreement
